Print an Application

You can print an application using the link below, then fill out and mail or fax the application along with the other required documents. A Fast Trak Temps, Inc. representative will contact you immediately after receiving your application.

All parts of the application must be completed, even if the information is repeated or is covered in your resume. If you do not have information for any part of this application state "None" or "N/A".

Please send the following information with your application:

• Copy of any State Professional Licenses (both sides).
• Copy of recent CPR Certifications.
• Copy of Professional Board Certification Card (both sides).
• Proof of recent Rubella vaccination and PPD screening or recent chest X-ray report.
• Copy of your Social Security Card, valid State Driver's License and Automobile Insurance Card.
• If you are not a U.S. Citizen, a copy of your green card.
• Two letters of professional reference.
